Chaplain program.
The first step is to be accepted into the UCFM Chaplain program,
as follows:
- Complete the Chaplain application below
- Pass Criminal Background Check by UCFM
After you fill out this application, we will
be in touch with you, in order to proceed
through the next steps mentioned on our home
page.
FORM INSTRUCTIONS: This first step of the application does require your driver’s license to be uploaded. Be sure you do that. After you fill out the first form and attach a copy of your driver’s license,
please
allow some time for a Coordinator to get in
touch with you after that.

This form does require a Google account. You can create a temporary account even if you don't use it in the future. We suggest to just sign up using your regular email address rather than getting a gmail address.
