IMPORTANT INFORMATION PLEASE READ!
This form is to assist locating a prior membership record which no longer is listed online (most likely is archived offline) and needs to be restored to online active status. Please, however if you are a past member with ordination credentials - do not apply for new membership. WE will gladly restore your old record with all the past information. Once we find your record we will restore it, and send you an email giving your username and password to login. After receiving this email you may go in and edit any necessary information.

Additionally, the UCFM has implemented a terms and conditions of ordination (TACOO) policy. This applies to all who are ordained in the UCFM and presents the current terms and conditions for your ordination which you must be in agreement with in order for your credentials to be in "good standing." If you have not yet read and agreed with the TACOO, these terms will be presented to you to read and agree with when you first login again. If however you do not agree with the terms and conditions of ordination (TACOO) your ordination credentials will not be restored to good standing.
