Un-needed names: Please do not use un-needed names. We don't need your maiden name if your Married.
Keep it simply your legal name.

Nicknames: No nicknames! Not John "Duke" Doe.

Format: First Name then Middle Name (or Initial) and then Last name. Example: John S. Doe

Jr. - Sr.: If you are a Sr. or Jr. please format your name as in this example: John S. Doe, Jr.

UPPER/lower Text: Use proper UPPER/lower case text, see example below.
Wrong: JOHN S DOE
Wrong: john s. doe
Correct: John S. Doe

Titles & Degrees: In the name field we are asking for your legal name - NOT your title or any degrees!
